problem with creating class

i am having problem in creating class in C++, every time i try to create class it says wizard cannot continue  ,try changing file name.  i did change file name plenty of times. but still same error. I reinstalled code blocks couple of times but still same error. please help

Why do you upload the same image three times ?
Why do you not upload a screen shot of the wizard-page (as requested) with the content you have entered ?
